Service navigation program helps older adults age in place — and it’s free (opens in new tab)

Nearly half of older adults require assistance with daily activities in a given month. Implemented at Yale’s Dorothy Adler Geriatric Assessment Center, the Agency on Aging of South Central Connecticut’s Service Navigator program addresses the complex needs of older adults and their caregivers.
