Designing effective CAR T cells is challenged by cellular heterogeneity and a vast design space. Here, we introduce CAROT, an optimal transport-based framework that predicts heterogeneous single-cell gene expression responses and generalizes to unseen CAR designs, enabling systematic exploration of CAR architecture-function relationships.
